Jade Community Care, Inc. is a Texas HHSC-contracted provider of Home and Community-based Services. We support adults with intellectual and developmental disabilities across Fort Bend County, Greater Houston, and all surrounding areas.

Why we exist
Jade was founded on a plain conviction: a person with a disability should get to live in a real home, in a real neighborhood, with people who know them — not warehoused, and not passed from one caseload to the next.
We started with one house. Today we operate several small group homes across the Richmond area and a network of screened, trained host home providers, and we serve more than sixty adults through the HCS waiver.
We have deliberately stayed small enough that our leadership knows every person we serve by name. That is not a marketing line — it is an operating constraint we accept on purpose, because it is the thing that makes the care good.

Four things we will not compromise on
The person decides
Person-directed planning is not paperwork to us. Where someone lives, who they live with, what they do during the day, and what they are working toward — those are their calls, supported by the people who love them.
A home, not a facility
Our houses are houses. People have their own rooms, their own things, a key to the front door, and a say in what is for dinner.
Safety you can verify
Background and registry checks on every staff member, nurse oversight of medications and health changes, documented incident reporting, and records that hold up to state review because they are accurate.
Families stay in the picture
Moving into one of our homes does not mean stepping out of someone's life. We keep families informed, invited, and welcome — and we answer the phone.
