Our Programs
Individualized Skills and Socialization
Most families still call it day hab. Texas renamed it ISS on March 1, 2023, and changed more than the name — the focus moved from a building people go to, toward skills, choice, and real time in the community. Jade provides it, and we have room.
- Texas HHSC-licensed day activity program
- On-site, community, and in-home
- Openings available now
- HCS, TxHmL, and DBMD
Lost your day hab program? Start here.
When Texana Center ended its waiver services in 2025, a lot of families in Fort Bend County were left without a day program and without much notice. We are still taking those referrals. If you are looking for ISS or day habilitation for your family member — whether you are with Jade for residential services or not — call us and we will tell you honestly and quickly whether we have a spot.
How We Deliver It
Three settings, usually mixed across the week
A person's week is not built around what is convenient for us. It is built from their Person-Directed Plan — what they want to get better at, and where they want to be while they do it. Most people we serve use more than one of these.
On-Site
Structured activities at our licensed day activity location. This is where the things that need equipment, routine, or a predictable room happen — cooking and kitchen safety, handling money, computer and phone skills, group activities, and the quiet space someone can step into when the day gets loud.
Off-Site
Out in the places anyone else goes: the library, a park, the grocery store, a gym, a museum, a volunteer shift. The measure of a good outing is not that it happened but that the person chose it and got something out of it.
In-Home
Delivered in the person's own residence. The right answer for someone whose health, behavior, or plain preference makes leaving the house hard, and for the skills that only make sense at home — laundry, cooking, managing a routine.
What a Day Looks Like
Not a room where everyone does the same thing.
Every activity is chosen with the person, tied to a goal in their plan, and documented against it — which is both what the state requires and the only way to know whether the program is doing any good.
Staff ratios are set by what people actually need, not by what fills a van. If someone needs one-to-one support to be out in the community, that is what gets scheduled.
- Skills that carry overCooking, money, transportation, self-advocacy, safety
- Real community participationPlaces anyone can go, not staged outings
- Work and volunteeringAlongside Employment Assistance and Supported Employment
- Health and safety oversightMedication support and nurse involvement carry into the day
- Transportation coordinationArranged as part of the plan
- Documentation that holds upEvery service hour tied to a goal and a signature
The Rules
How much ISS a person can receive
Texas HHSC sets firm limits on ISS, combined across all three delivery types:
| Limit | Amount |
|---|---|
| Per IPC year | 1,560 hours |
| Per calendar day | 6 hours |
| Per calendar week | 5 days |
A person's approved hours are written into their Individual Plan of Care, so the actual amount varies. On-site and off-site ISS must be delivered by a provider holding a Day Activity and Health Services – Individualized Skills and Socialization license for that location; in-home ISS does not require one.
Jade holds the license.
Our day program is licensed by Texas Health and Human Services as a Day Activity and Health Services – Individualized Skills and Socialization provider, so we can deliver all three forms of the service: on-site at our location, off-site in the community, and in the home.
Who It Is For
Available through three waiver programs
ISS is available to people enrolled in:
- Home and Community-based Services (HCS)The residential program Jade also provides
- Texas Home Living (TxHmL)A smaller waiver for people living with family or on their own
- Deaf Blind with Multiple Disabilities (DBMD)For people with deaf-blindness and additional disabilities
You do not have to live with us to come to our day program.
Plenty of the people in our ISS program live at home with family or with another residential provider. ISS is a separate service in the plan — you can choose Jade for it on its own.
Getting Started
How to join the program
Call us
Tell us who the person is, what waiver they are on, and roughly what they are looking for. We will tell you whether we have room. (713) 447-0726
We talk with your service coordinator
ISS gets added or moved in the Individual Plan of Care. If you are transferring from another provider, this is the step that does it, and we handle most of the paperwork.
Come see the program
Bring the person. They should get a look at the place and the people before anyone signs anything.
Build the schedule
Which days, how many hours, and what mix of on-site, community, and in-home. It can change as goals change.
Want to work on the ISS team?
Skills and socialization staff spend their days out in the community helping people build real independence. If that sounds like your kind of work, we would like to hear from you.
